CUSTOMER AGREEMENT

By signing this form, I certify that I have purchased a composting system for residential use at the address indicated above within the City of Tamarac.  I understand that in order to receive this rebate, I must read and agree to the Terms and Conditions of this program. All statements made in this application are correct. I understand that the City of Tamarac reserves the right to inspect and verify any equipment before issuing rebates. I will provide a photograph showing the composter being used.  I verify the information to be factual.
